REQUIREMENTS

A fee of $75.00 will be charged. Establishment not to operate more than 7 consecutive days. Establishments that are for charity, church or public welfare are exempt from fee, but ARE subject to all other provisions. Permit is to be posted in view of customer.
Only listed foods & beverages may be authorized.
No home preparation allowed.
If needed.

MUST BE PRESENT TO OPEN FOR OPERATION

*Metal Stem Thermometer, *Invoices for purchase of foods from approved source, *Bleach, *Handwash facility - hand soap and paper towels, *Disposable gloves(can supplement with hand sanitizer-gel), * Hair Restraints, *Approved water source, *Floor covering (if required), *Utensil washing facility, * Proper wastewater disposal, * 3 Wash-Rinse-Sanitize bins * Overhead canopy/covering (where needed) * Fire extinguisher (Fire Dept Approved). ALL STAFF PREPARING FOOD ARE REQUIRED TO HAVE AN APPROVED CERTIFIED FOOD HANDLERS COURSE CERTIFICATE.
